Who can fill out the Certificate of Health?

The Certificate of Health form is a general health form that must be filled out by a Doctor of Medicine (M.D.) or General Practitioner (G.P.) preferred. A Doctor of Osteopathic Medicine (D.O.) or Nurse Practitioner (N.P.) can fill out the form only where authorized. For Early Arrival participants, the Certificate of Health is not due by Friday, November 13. The form is due in January 2027 after proceeding to the second stage of the application process and before your interview is conducted. Main Arrival / Summer candidates do not need to submit this form until after official acceptance into the 2027 Program in spring 2027.
